Agreements and Authorizations
I understand that UUFC assumes no liability for loss, damages, or any kind of injury sustained by my child, myself, or our property while participating in youth events. I therefore assume all risk associated with participation in youth activities. By signing this release of liability and participating in UUFC youth events, I hereby fully and forever release and discharge UUFC and their staff, volunteers, and agents from any and all claims or demands resulting from or arising out of my participation in UUFC events.
I do hereby authorize the staff and volunteers of UUFC to consent to any necessary emergency transportation, examination, anesthetic, medical diagnosis, surgery, or treatment and/or hospital care to be rendered to the above-named minor, under the general or special supervision and on the advice of any physician licensed to practice medicine. I understand that I will be responsible for all financial obligations and insurance claims resulting from the aforementioned care. I understand that this consent is allowed to emergency treatment to be initiated without delay and that the staff, volunteers, and emergency personnel will continue efforts to contact me.
Nursery Guidelines Acknowledgement
1. While we seek to minimize all risk to children in our care, accidents do occur. Any participant in UUFC children and youth spaces acknowledges these risks and assumes all responsibility.
2. Parents or legal guardians are responsible for the supervision of their children and youth outside of designated fellowship children's or youth activities. Children under 18 are not permitted to linger unattended in the classroom wing or playground.
3. Nursery staff and volunteers are not authorized to change diapers. In the event of a necessary diaper change, you will be texted at the number provided above. Please make sure your phone will buzz while you're in the sanctuary.
4. Parents may drop their child off in the nursery no more than 15 minutes prior to the start of service, and agree to pick them up before attending coffee hour or additional activities.
5. Parents may request free childcare for any UUFC-sponsored event by contacting Skyla King-Christison for a Grandfolks Squad Referral.
6. Parents must register their child once per "church year," and once registered, children will be checked in by our nursery volunteers and staff each week. Attendance records are kept in our database. Should we learn that your child has come into contact with a communicable illness during their time in the nursery, we will use the information in this form to contact you.
7. Photos of participants may be used by UUFC unless consent is withdrawn. Consent may be withdrawn by contacting Skyla King-Christison (dre@uucorvallis.org)
