Hi, Parents! This registration form is for children and youth of all ages who wish to participate in Sunday morning Religious Exploration programs, as well as for youth visiting from another congregation for a youth event.

Please fill out one form per child. This form is for the 2026-27 Fellowship year, starting in June of 2026. Even if you just registered your child in May, you will be asked to fill out this new form for the new year. Thank you for helping us keep our records up to date, and your children well supported!

Shared Ministry

Our programs thrive on collaborations between staff, families, and other dedicated adults in the congregation, but parental support is key! We hope that every parent who benefits from our programs will also find some way to participate in them!

If you are new to us, please take some time to get to know us and settle in. If you have been with us for 3 months or longer, we hope that you'll join our program in a way that suites your unique skills and gifts. 

All RE collabroators receive training and invitation to team-building and spiritual gatherings intended to fill the cups of those who pour themselves into our vital programs.

Acknowledgements and Authorizations

I understand that UUFC assumes no liability for loss, damages, or any kind of injury sustained by my child, myself, or our property while participating in Fellowship events. I therefore assume all risk associated with participation in Fellowship activities. By signing this release of liability and participating in UUFC events, I hereby fully and forever release and discharge UUFC and its staff, volunteers, and agents from any and all claims or demands resulting from or arising out of my family's participation in UUFC events.


I do hereby authorize the staff and volunteers of UUFC to consent to any necessary emergency transportation, examination, anesthetic, medical diagnosis, surgery, treatment, and/or hospital care to be rendered to the above-named minor, under general or special supervision, and on the advice of any physician licensed to practice medicine. I understand that I will be responsible for all financial obligations and insurance claims resulting from the aforementioned care. I understand that this consent allows emergency treatment to be initiated without delay and that the staff, volunteers, and emergency personnel will continue efforts to contact me.

RE Rules and Policies

The following are a combination of RE rules and some of the UUFC Board Policies that we hope will help your family navigate Fellowship life with ease. 1. Parents, legal guardians, and other adults who bring minors to Fellowship property are responsible for the supervision fo their children and youth outside of designated RE programs and activities. Children under 18 are not permitted to linger unattended in the classroom wing or playground, and must be accompanied by a supervising adult while on UUFC property. 

2. No alcohol, tobacco, vape, and drugs are not permitted at any children or youth-oriented gatherings.

3. No sexual activity or explicitly sexual media of any kind are permitted at children's or youth-oriented gatherings. While we may discuss sexuality in an open and honest manner, interpersonal sexual expressiong does not support the welcoming and safe environment we aim to share. 

4. Intentional destruction of personal or Fellowship property is if forbidden, including but not limited to grafitti, stickers, and carving. It is understood that everyone involved in Fellowship activities will take reasonable care of the environment and belongings in the space. 

5. Violent language and behavior are not permitted on UUFC property. 

6. Firearms, and weapons of any kind are not permitted on UUFC property. Pocket knives and the like should be left at home for RE gatherings.  

7. Children and youth may not leave a designated group activity without permission. What consitutes permission  may vary by event but must require permission from the supervising adult. 

8. Rule of Threes: All multigenerational gatherings and excursions must include a minimum of three participants. Related individuals canot satisfy this requirement. 2 unrelated adults must be present wherever minors are in our care without their guardians.


9. Parents consent to being contacted by the RE program via text and email unless consent is withdrawn by email to dre@uucorvallis.org

10. Board policy prohibits the climbing of trees on UUFC property, even with permission and adult supervision.
